Thank you for considering becoming a client at Mt. Tabor Psychological Services. Dr. Martindale is currently open to new clients. However, there may be a significant wait to get an appointment that best suits your schedule. Late afternoon and evening hours tend to be the most requested and therefore the least available. We hope to get you scheduled as soon as possible. Please feel free to call our office for current information on scheduling. Dr. Martindale is particularly interested in working with individuals who share her values and those who desire to create a meangful and passionate life.
In order to schedule your first session we will need you to fill out and return to us the following forms. Please download these forms using the links below. If you prefer, you can call our office to have these forms mailed to you.
The forms are Adobe PDF (Portable Document Format). You may download the free reader from Adobe to open the documents. You may type the information requested into the forms or you may print the forms and fill them out entirely by hand. Some parts of the forms will require your responses by hand (symptom sheet, consent forms requiring signatures, and scheduling information) so even if you type in most of the information, you will still need to print out the forms to complete them. All the information in these forms is held in the strictest confidence. The information is used to aid us in developing a plan to best meet your objectives in therapy. The information on the Insurance Information Form assists us in billing your insurance company.
Additionally, you may find it helpful to think about what you want to get from your therapy investment. The form below can help you think this through. You can send it along with your other paperwork if you desire.
